How to get your Ayushman Bharat card

Five steps

• 1. Check eligibility — on pmjay.gov.in ("Am I Eligible") or call 14555 with your ration-card or mobile number.
• 2. Documents — Aadhaar, ration card / family ID, and mobile number.
• 3. Enrol — at any empanelled hospital's Ayushman desk, Common Service Centre (CSC), or via the Ayushman app with Aadhaar e-KYC.
• 4. Download the card — from the app or beneficiary portal once approved.
• 5. Use it — cashless treatment up to ₹5 lakh per family per year at empanelled hospitals.
